Welcome to the Faculty Service Center
The National Alliance for Care at Home thanks you for agreeing to serve as faculty for the 2025 National Alliance for Care at Home Annual Meeting & Exposition. Specific session dates and times will be sent in a couple of weeks, so please maintain flexibility to present during the conference period of Monday, November 3 through Tuesday, November 4.
To support a smooth and successful experience, we’ve created the Faculty Service Center (FSC)—your go-to hub for important information, required tasks, and helpful resources related to your role. We hope you’ll find the FSC intuitive, convenient, and easy to navigate as you prepare for the event. Thank you again for your time, expertise, and commitment to advancing care at home.

What Happens When Sales and Admissions Finally Align
Monday, November 03, 2025: 11:00 AM - 12:00 PM
Speaker(s)
Description
In this session, Delaware Hospice and Transcend Strategy Group will share how a cross-functional admissions improvement project led to faster patient access, stronger internal alignment, and an 11% increase in daily census – in just 60 days. Together, we’ll walk through a practical framework for diagnosing and fixing breakdowns in the referral-to-admission process, including six critical focus areas: referrals, eligibility, admissions logistics, patients not taken under care (NTUC), people and tech. Learn how Delaware Hospice brought together sales, intake, nursing and leadership in a unified effort to reduce friction, establish daily operational rhythms and build shared ownership across teams. Attendees will leave with actionable strategies for building a high-performing admissions engine that supports both mission and growth.
