The National Alliance for Care at Home thanks you for agreeing to serve as faculty for the 2025 National Alliance for Care at Home Financial Summit

To support a smooth and successful experience, we’ve created the Faculty Service Center (FSC)—your go-to hub for important information, required tasks, and helpful resources related to your role. We hope you’ll find the FSC intuitive, convenient, and easy to navigate as you prepare for the event. Thank you again for your time, expertise, and commitment to advancing care at home.


Faculty Action Items

  • Review and complete the 2025 Alliance Fianncial Summit Faculty Disclosure form by May 21.

  • Registration is open! To receive your faculty rate, enter your exclusive code 2025FACULTY on the Profile Page next to “Discount Code.” Note: This code is for faculty use only and may not be shared.

  • Book your hotel: Visit the Hotel and Travel Information page to secure your accomodations at the Sheraton Grand Chicago Riverwalk. 

  • My Profile Update: Please make sure all your personal information is correct in this section, and make changes if needed. You will also notice the “Faculty Bio” at the bottom of the page. The information provided here will be posted on the event website and reprinted in event-related materials, so please double check this for accuracy, and revise as needed. 

  • Faculty Prep Call:  Schedule time with event planners using the Bookings link to speak with the Alliance team regarding your session. The call will last under 30 minutes and is your chance to ask questions, hear feedback, or brainstorm new and creative ideas to make your session great.

  • Faculty Office Hour:  Click below to reserve your spot in faculty office hours. Come prepared with questions or join and listen to other faculty share their best practices. Click below to reserve your spot:

  • Submit Session PowerPoint: Email your completed PowerPoint presentation to faculty@allianceforcareathome.org. This allows the Alliance Education Team to review and ensure accreditation standards are met.

Review Steps
  • When ready to begin reviewing, select the “Proposal Title” below.
  • Read the proposal.
  • Below the proposal, you will see the rating criteria. Choose the score you feel is most applicable based on the criteria.
  • In the Comments field, note any overlap or synergies with other proposals you have reviewed.
  • Once you have scored all criteria, select "Submit" at the bottom of the proposal page.
  • The page will reload and the proposal status will change to "Reviewed".
  • Review Deadline: April 10, 2025 at 11:59 pm ET

Continuing Education/Continuing Medical Education (CE/CME)  

Continuing education credits/hours are pending and subject to change upon finalization.

The 2025 National Alliance for Care at Home Financial Summit will offer CE/CME credit for interprofessional, nurses, physicians, and continuing professional education for accountants for those who attend the live event in Chicago, IL.

The material presented in the education sessions represents the opinions of the faculty and not necessarily the views of the National Alliance for Care at Home, the Accreditation Council for Continuing Medical Education (ACCME), the American Nurses Credentialing Center (ANCC), or the Association of Social Work Boards (ASWB). 
 

ACCREDITATION STATEMENT

In support of improving patient care, the National Alliance for Care at Home (the Alliance) is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.  

Credit hours are determined based on individual attendance/participation and evaluation of each session and may be less than the maximum number offered. In addition, the amount of CE/CME credit available is subject to change.  

Joint Accreditation IPCE Credit Statement 
This activity was planned by and for the healthcare team, and learners will receive a maximum of TBD live learning activity Interprofessional Continuing Education (IPCE) credit for learning and change. 

CNE Credit Designation Statement 
The Alliance designates this live learning activity for a maximum of TBD nursing contact hour(s). Nurses should claim only the contact hours commensurate with the extent of their participation in the activity. 

CME Credit Designation Statement 
The Alliance designates this live learning activity for a maximum of TBD AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. 

Certificate of Participation for Non-physician Healthcare Professionals 
For participants whose disciplines are other those above may request a Certificate of Participation for Non-physician Healthcare Professionals to submit to your accrediting bodies/licensing boards for continuing education credit. This certificate will indicate this activity was certified of TBD AMA PRA Category 1 Credit(s)™. It is the responsibility of participants to submit the certificates to their accrediting bodies/licensing boards for approval. Unfortunately, the Alliance cannot guarantee its acceptance. To receive this certificate, you must complete all CE/CME requirements and indicate your desire to receive this certificate on the evaluation form.  

Continuing Professional Education for Accountants
The National Alliance for Care at Home sociation for Home Care & Hospice is registered with the National Association of State Boards of Accountancy (NASBA) as a sponsor of continuing professional education (CPE) on the National Registry of CPE Sponsors. State boards of accountancy have final authority on the acceptance of individual courses for CPE credit. Complaints regarding registered sponsors may be addressed to the National Registry of CPE Sponsors, through its website: www.NASBARegistry.org.  

Credit amounts are pending approval and may change. Each education session provides 1.2 CPE for accountants based on 50 minutes. Accountants can earn up to 9.6 CPEs for this live activity.
