Serving individuals with both Medicare and Medicaid eligibility (referred to as Dual Eligibles) poses unique challenges and opportunities for providers.         As we continue a deep transition from the majority of patients being Traditional Medicare payers to being enrolled in a Medicare Advantage Plan, Home Health Agencies must be effective at managing each and every patient episode from accurately confirming the payer to authorizations, caring for the patient and collecting payment. The Home Health Revenue Cycle begins at intake and each payer must be dealt with individually. In addition, the new CoP, Acceptance-to-Service Policy requires that agencies document the process of evaluating and accepting patients into their home health care.           The HOPE tool has been finalized for implementation October 1, 2025, as a new required instrument for standardized data collection contributing to the HQRP. HOPE will replace the HIS and expand the time points for documentation requirements that CMS will utilize to collect data to enhance the HQRP and potentially be used to in future payment refinements. Additional time collection time points, beyond admission and discharge known as HOPE Update Visits (HUV) may now be required, dependent on LOS and patient symptom control required follow up that all will impact clinical and operation staff for scheduling, completion and submission. The submission requirements for the HOPE tool are mandatory and will result in a 4% payment penalty for providers not meeting the 90% timely submission threshold.
